Swing Bed
Our Swing Bed Program provides rehabilitation and recovery services for patients who need time to rebuild strength following illness or surgery. The goal is to help patients return home and live independently or with the help of community resources. To qualify for this Medicare-sponsored program, patients much have a three-midnight inpatient stay at any hospital and must require skilled care such as IV therapy, physical therapy, occupational therapy, or speech therapy.
Swing Bed/Skilled Care services include:
Rehabilitation, such as physical therapy, occupational therapy, and/or speech therapy
Round-the-clock nursing care
Activities
Discharge planning team
Weekly meetings with patients and family
Nutritional guidance and counseling
Wound care management
In-home evaluations (if needed)
